General Information:

Jeremiah often wrote prophecy in the form of poetry. Hebrew poetry uses different kinds of parallelism. (See: rc://en/ta/man/translate/writing-poetry and rc://en/ta/man/translate/figs-parallelism)

The sin of Judah is written ... on the horns of your altars

Yahweh speaks of Judah's sins being so great that they cannot stop committing them as if someone had engraved their sin permanently. (See: rc://en/ta/man/translate/figs-metaphor)

The sin of Judah is written

Here the word "Judah" represents the people of Judah. This may be expressed in active form. AT: "Someone has written the sin of Judah" (See: rc://en/ta/man/translate/figs-activepassive)

It is engraved

engraved on the tablet of their hearts

The sinful habits of the people are spoken of as if their sins had been engraved on their own hearts. The word "hearts" refers to the whole person: their thoughts, emotions, and actions. AT: "engraved in their very beings" (See: rc://en/ta/man/translate/figs-synecdoche)
